Please provide at least 48 hours notice to reschedule or cancel appointments,
otherwise we will assess fees up to 100% of the anticipated visit.
Dr. Kate wants appointments for all provider related-requests outside of your regularly scheduled visits.
This will include adjustments to your custom hormone therapy and other considerations that require reviewing your history and making medical decisions and changes to your current regimen.
Finally, DO NOT UPLOAD ANY FILES OR DOCUMENTS UNLESS WE SPECIFICALLY ASKED FOR THEM.
